The Luxe Clinic is committed to protecting the privacy of your health information. This Notice describes how we may use and disclose your Protected Health Information (PHI) and your rights regarding that information.

We are required by law to maintain the privacy of your PHI, provide you with this Notice of our legal duties and privacy practices, and follow the terms of the Notice currently in effect.

How We Use and Disclose Your Health Information

Treatment: We may use and share your PHI to provide, coordinate, or manage your care.

Payment: We may use and share your PHI to bill and collect payment for services.

Health Care Operations: We may use and share your PHI to run our practice, improve services, and conduct quality reviews.

As Required by Law: We will disclose your PHI when required to do so by federal, state, or local law.

Public Health Activities: We may disclose your PHI to public health authorities as required by law.

Law Enforcement: We may disclose your PHI in response to a court order, subpoena, warrant, or similar process.

Your Rights Regarding Your Health Information

Right to Inspect and Copy: You have the right to inspect and receive a copy of your PHI. Submit a written request to our office.

Right to Amend: If you believe your PHI is incorrect or incomplete, you may request an amendment. We may deny your request under certain circumstances.

Right to an Accounting of Disclosures: You may request a list of disclosures we have made in the last six years, other than for treatment, payment, or health care operations.

Right to Request Restrictions: You may request restrictions on how we use or disclose your PHI. We are not required to agree.

Right to Confidential Communications: You may request that we communicate with you in a specific way or at a specific location.

Right to a Paper Copy: You have the right to receive a paper copy of this Notice at any time.

Changes to This Notice

We reserve the right to change this Notice at any time. Any revised Notice will be effective for all PHI we maintain. The current Notice will always be posted in our office and on our website.

How to File a Complaint

If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
